YOUR CHILD IS NOT CONSIDERED REGISTERED UNTIL A PAID REGISTRATION FORM,THE 3 MANDATORY FORMS, AND YOUR CHILD IS FITTED FOR A UNIFORM (IF NEEDED) ARE ALL DONE. New for 2011....All cheerleaders must turn in their paid registration form, uniform form, special request form, and code of conduct form before May 31St, 2011 to be considered fully registered in our program. After May 31St, those that did not turn in their forms will be removed from our rosters and issued a refund minus the $25 SPORTS Office fee. Please fill out separate forms for each Cheerleader. FILLING OUT ONLY THE 3 FORMS DOES NOT MEAN YOU ARE REGISTERED...YOU MUST STILL HAVE A PAID REGISTRATION FORM AND BE FITTED FOR A UNIFORM (IF NEEDED)TO BE COMPLETELY REGISTERED.
